  • Ship equipment according to the directions on the form.
  • A completed Repair Service Form must be included. Equipment shipped without a form will be returned.
  • Warranty status and a repair estimate will be provided for your approval.
  • A $60 evaluation fee applies to non-warranty items if you choose not to proceed with the repair.
